How It Works
Help Me Grow is a free family support line that serves children and their families from pregnancy through kindergarten. We connect families to referrals and local resources related to child health, behavior, development and learning. We are the early childhood specialist team of Swindells Resource Center. Together, we are able to provide a continuum of care for families prenatal through adulthood. Our phone number and contact form will get you to either team.
We support you and your office by:
Providing child development information and parent skills coaching
Navigating community resources for your patients
Partnering for more connected services for your patients
Helping patients connect to their medical or early intervention referrals
Refer a Patient
If you would like us to update you on the status of a referral, we will need a signed Release of Information. After a family has been referred, we will send confirmation of the received referral. Our resource navigation team will follow up with the family within 3 business days. With the family’s consent (the release of information forms below), we will keep you updated on the status of your referrals.
Ways To Refer
Print the Release of Information form (below), obtain signature of parent and provider, and fax to Help Me Grow at 503-487-3585.
Providers and families can self-refer by calling 833-868-4769.
Providers and families can also fill out our contact us form. *Please note, without a signed ROI, we will not be able to update providers on the status of referrals.
